Model for Change: A comprehensive approach to health equality that addresses underlying causes and engaging the community to work together to effect change.
Nutrition and Fitness: Promote opportunities for healthy lifestyles by providing direct education, implementing healthy policies in institutions and build healthy communities with health choices and active lifestyles.
Education and Outreach through partnerships with community members, local organizations and faith-based institutions: Primary focus is diabetes, healthcare access disparity, nutrition and fitness. Health activities that are organized by faith-based institutions include: Fine, Fit, and Fabulous, a nutrition and fitness program; The Culinary Initiative, a healthy cooking course for culinary committee members; God’s Health Squad, a youth nutrition program; and The Way, a support program for congregants living with diabetes.
Policy and System Change: Educate policymakers and legislators about promoting accessible and high quality models of care and addressing the environmental factors that contribute to health disparity. Meeting with local elected officials to advocate for change in health policy and to make quality healthcare more readily available and equitable in underserved communities. Raise awareness in schools, churches, and communities on how to implement nutrition and fitness policies that promote healthy eating, physical activity and access to fresh, affordable food.
