Long Organization Description:

The mission of Bridging Access To Care, Inc (“BAC”) is to reduce the incidence of HIV in Brooklyn through information and education to ensure that the message reaches the most vulnerable individuals. The corporation primarily provides preventative, educational and direct services to the general population of the borough of Brooklyn and to people with AIDS and their families.

About the Organization:
Founded in 1985, BAC has since provided HIV/AIDS education, prevention and treatment, including HIV primary care and counseling and testing, as well as state licensed substance abuse treatment services and prevention services. BAC’s comprehensive services include housing placement assistance/living skills education and COBRA case management; treatment education and adherence services; nutritional services, mental health screening, case management, youth services, and employment services; risk- reduction/harm reduction services; adolescent peer education; and several behavioral modification programs targeting ex-offenders and/or recently incarcerated individuals with long histories of substance abuse.
